About the South Dakota Diabetes Coalition (SDDC)

Provider Survey

The South Dakota Diabetes Program is part of the South Dakota Department of Health, Office of Chronic Disease Prevention and Health Promotion. The program receives funding from the U.S. Centers for Disease Control and Prevention to use prevention and population-based intervention aimed at South Dakotans at risk for and with diabetes.

  • Facilitate partnerships to advance the South Dakota Diabetes State Strategic Plan in collaboration with the South Dakota Diabetes Coalition, healthcare systems, communities, organizations, and other stakeholders.
  • Collect, analyze, and disseminate diabetes surveillance and evaluation data to inform program development and guide policy decisions.
  • Support efforts to reach disparate and high-risk populations affected by diabetes across South Dakota.
  • Coordinate the development and implementation of population-based community interventions.
  • Provide accurate, current diabetes education materials and resources to healthcare professionals.​

About the South Dakota Department of Health Diabetes Program

The South Dakota Diabetes Coalition is a non-profit organization comprised of many diverse partners - healthcare professionals, advocacy groups, government agencies, tribal health representatives, persons with diabetes, quality improvement and wellness program coordinators, and many others. The South Dakota Diabetes Coalition is a volunteer-based organization comprised of professionals and citizens who are passionate about preventing diabetes and empowering those with diabetes to successfully manage their health.

  • Provide education, training, and resources for healthcare professionals and community partners.
  • Lead statewide collaboration to prevent and manage diabetes through partnerships with healthcare providers, public health agencies, community organizations, and other stakeholders.
  • Support patients and family members with diabetes resources and materials to manage their diabetes.
